Community Health Needs Assessment (CHNA) Survey
About the Survey
The Community Health Needs Assessment (CHNA) Survey, also referred to as the Community Health Assessment (CHA) Survey, is a comprehensive data collection effort to identify the strengths, challenges, and opportunities to address and improve public health. Every three years, Maricopa County Public Health, in collaboration with community organizations, municipalities, and healthcare partners, conducts a survey to develop a deeper understanding from residents about issues related to quality of life, availability of essential services, physical and physical and mental health, and more. The information gathered from the survey facilitates planning and guides the work toward making Maricopa County a healthier place for all.
2023 CHNA Survey Progress Report: How many surveys have been collected so far?

Since the survey launched on March 1, 2023, we have received over 12,900 responses from Maricopa County residents!
Please note, these numbers are subject to revision based on additional data entry and data cleaning.

The survey is open to Maricopa County residents ages 12 and up. Minors ages 12-17 must have parent/guardian verbal or written approval to take the survey.
Please note that residents are considered individuals that live in Maricopa County at least 6 months out of the year (e.g. college students, snowbirds).
Yes, information collected in this survey will be kept confidential and used only in combination with others participating in the survey. No personal identifying information, like name, date of birth, or phone number, will be collected. Your answers cannot be traced back to you.
Demographic questions such as your zip code, age, and race/ethnicity helps us identify how issues may affect some groups of people more than others and informs us where interventions and resources are most needed. In previous surveys, an increase in survey responses allowed us to divide up responses based on zip codes into five regions across the county. With the size of Maricopa County, this regional approach allows for resources to be focused on more localized community issues.
It is recommended that unsheltered/unhoused people use the zip code of the place they stay in most often. If they travel around the county often or don’t know the zip code where they stay, they may use the zip code of the place where they are filling out the survey.
Your survey responses will be used to help guide future community health improvement planning and funding efforts. In the past, survey findings have been used to share data with major hospitals and healthcare providers to inform healthcare delivery and direct funding to local organizations to address top health needs in the community. Results will be available to the public as well as cities and community-based organizations for their own planning and action purposes on our website at maricopahealthmatters.org. We anticipate survey results being available later this year.

This survey is open to Maricopa County residents ages 12 and up and takes about 15 minutes to complete. The survey will be open from March 1 - June 21, 2023.
Information collected in this survey will be kept confidential and used only in combination with others participating in the survey. No personal identifying information will be collected.
