Community Health Needs Assessment (CHNA) Survey
About the Survey
The Community Health Needs Assessment (CHNA) Survey, also referred to as the Community Health Assessment (CHA) Survey, is a comprehensive data collection effort to identify the strengths, challenges, and opportunities to address and improve public health. Every three years, Maricopa County Public Health, in collaboration with community organizations, municipalities, and healthcare partners, conducts a survey to develop a deeper understanding from residents about issues related to quality of life, availability of essential services, physical and physical and mental health, and more. The information gathered from the survey facilitates planning and guides the work toward making Maricopa County a healthier place for all.
The Survey is now closed. Thank you for taking it!
The 2023 Community Health Needs Assessment (CHNA) Survey is now closed. Thank you for taking the survey and sharing it with your friends, family, and the communities you work with. Over 18,000 Maricopa County residents responded to share feedback on how to improve quality of life and well-being in their community.
How many surveys have been collected so far?

When will results become available?
We anticipate preliminary survey results becoming available at the end of this year, and the final CHNA report published by Spring 2024. Our team is working diligently to finalize data entry & cleaning and survey analysis by Fall 2023. While the survey is a significant component of the CHNA process, there are other data collection efforts we utilize to better understand the community’s health experiences. From July through October 2023, we will continue data collection through focus groups, key informant interviews, and SWOT analysis.

CHNA survey collection is a huge effort that relies heavily on support from staff, volunteers, and our partners in the community. We thank everyone who supported tabling and outreach efforts to ensure all residents felt empowered to share their feedback on improving quality of life in their community.
