Community Health Needs Assessment (CHNA) Survey

The Community Health Needs Assessment (CHNA) Survey, also referred to as the Community Health Assessment (CHA) Survey, is a comprehensive data collection effort to identify the strengths, challenges, and opportunities to address and improve public health. Every three years, Maricopa County Public Health, in collaboration with community organizations, municipalities, and healthcare partners, conducts a survey to develop a deeper understanding from residents about issues related to quality of life, availability of essential services, physical and physical and mental health, and more. The information gathered from the survey facilitates planning and guides the work toward making Maricopa County a healthier place for all.

The Survey is now closed. Thank you for taking it!

CHNA Roadmap: when can we expect results?

Our team is working diligently to finalize data cleaning and analysis through early 2024.

While the survey is a significant component of the CHNA process, there are other data collection efforts we utilize to better understand the community’s health experiences. In early 2024, we will publish a report summarizing data from over 40 focus groups conducted in 2023. Additionally, we will conduct key informant interviews which will explore community strengths and assets, the built environment, and forces of change.

Throughout the year, we will be sharing data and reports featuring findings collected from CHNA data collection efforts. To be notified when data and reporting becomes available, please click here to sign-up to the CHNA email list.

CHNA survey collection is a huge effort that relies heavily on support from staff, volunteers, and our partners in the community. We thank everyone who supported tabling and outreach efforts to ensure all residents felt empowered to share their feedback on improving quality of life in their community.
