PPE
Personal Protective Equipment (PPE) requests can include N95 respirators, surgical masks, face shields, and isolation gowns.
If you are a Private Practice or Private Doctor’s Office please go to the Maricopa County Medical Society for requesting PPE. Click here for their Request Form
Before submitting a PPE request you must prove that you have taken the following steps:
- Follow CDC’s guidance on optimizing PPE Supplies.
- Reach out to multiple vendors. If vendors cannot supply your order, keep the documentation.
- Reach out to other partners in the area. Many jurisdictions are already sharing with each other.

COVID-19 Testing Kits
The CDC recommends using authorized nucleic acid or antigen detection assays that have received an FDA EUA to test persons with symptoms when there is a concern of potential COVID-19. Tests should be used in accordance with the authorized labeling; providers should be familiar with the tests’ performance characteristics and limitations.
Before submitting a request for COVID-19 Testing Kits you must prove that you have taken the following steps:
- Review CDC’s Overview of Testing for SARS-CoV-2. Please make sure you are testing in accordance with CDC guidelines.
- Reach out to multiple vendors. If vendors cannot supply your order, keep the documentation.
- Reach out to other partners in the area. Many jurisdictions are already sharing with each other.

Request Ventilators
During a large-scale public health emergency involving a respiratory disease like COVID-19, stockpiled ventilators will be deployed in a way that optimizes the effectiveness, efficiency, and equity of this scarce resource.
Before submitting a request for Ventilators you must prove that you have taken the following steps:
- Review CDC’s Strategies to Allocate Ventilators from Stockpiles to Facilities. This guidance details how ventilators will be strategically and ethically distributed among healthcare facilities in Arizona during COVID-19
- Determine your need. Consider the number of and preferred type of ventilator. Please refer to the Ventilator Request Guidelines for a complete list of considerations.

Request COVID-19 Vaccine
COVID-19 vaccine requests must be made directly through ASIIS. Please follow this link for more information about How to Order in ASIIS. Pfizer 12+ (gray cap), Pfizer 5-11, Moderna and Janssen are all available for ordering directly in ASIIS. We also encourage you use the Matchmaker Tool to request transfers from other facilities in Maricopa County and help reduce wastage.

Request School Provisional Staffing Support
The Provisional School Staffing program is a COVID-19 emergency response initiative designed to provide health office support staff to underserved schools. As the product of a health-inequity grant, this program prioritizes provisional school staffing through a COVID-19 prioritization rubric based on the needs of both the school and its surrounding community.
