Step 1 - Prepare Your Documents
To avoid delays, prepare the following documents before completing this application form. Please upload the required documentsbefore submitting your request:
If ordering a certified copy of an AZ birth record for someone other than yourself or the parents listed on the birth certificate, you must also upload a copy of proof of your relationship to the person named on the birth record or provide a copy of proof of eligibility.
Step 2 - Complete the Automated Application
When completing the application, always check and make sure all the information is filled out correctly.
Once your request is received, an EMAIL notification receipt will be sent to your email address with a tracking number.

Please allow 3-5 business days for application processing time and then time for US Postal delivery.
Once you have submitted the application and provided your return telephone number, one our team members will contact you and discuss the status of your request and collect your fee. Please be prepared to pay the certified copies fee. All major types of credit cards (Visa, Mastercard, Discover, AMEX) are accepted.
If you meet eligibility, your ordered certified copies will be delivered using the US Postal Service (USPS).
