Welcome! We offer same day, walk-in service at 4 locations across the valley from 8:00 am to 4:30 pm daily. Please arrive by 4 pm for service.
You may apply for and purchase certified copies of Arizona birth and death certificates, apply for or learn how to submit legal corrections to existing Arizona birth and death records, register a home birth, and learn more about private burial. We are happy to assist you. 

We have gone paperless! Please complete your application on our electronic tablets located in the lobby. Paper applications will no longer be accepted at our locations.

Upcoming Office Closures:

  • Thursday, November 24 and Friday, November 25: All locations will be closed both days for the Thanksgiving holiday.
  • Thursday, December 8 (half day): All locations will be closed 8 a.m. - 12 p.m. and will resume regular hours at 1 p.m.
Each state maintains its own vital records. If you need a birth or death record from another state, contact that state's vital records office. Birth and death records for events that occur outside of Arizona are not available from Maricopa County Vital Records.
