Welcome to the Maricopa County Department of Public Health's  (MCDPH) Office of Vital Registration, Medical Certifier resource and information page.

We have developed this to aid you and your staff in preparing the necessary information for a decedent's death certificate.

Important Update!

In October 2016, the State of Arizona  revised the rules that govern how vital records are administered.  Please take a few moments to read carefully  as information has been revised and there may be differences and or updates to the process you will follow to input medical certification of death.  According to the state bureau, they will continue to provide information pertaining to these processes over the next few months.

Please return to this site frequently as the state will also be launching a new electronic death registration system planned for debut in the fall of 2017.  There may be additional data entry requirements for you to follow and online trainings offered by their offices.

We will continue to share information and resources as we learn them.
