Health Care for the Homeless
Health Care for the Homeless (HCH) provides health care services for homeless individuals and families throughout Maricopa County. We deliver quality health care to homeless people through street outreach, integrated primary care, mental health services, substance abuse services, case management, and nutrition services.

Human Services Campus (HSC)
The Health Care for the Homeless clinic is located on the Human Services Campus (HSC) in downtown Phoenix. The HSC represents a regional approach to addressing the needs of the homeless population. In conjunction with the Maricopa Association of Governments, representatives from the public and private sectors have come together to identify the unmet needs within the homeless population. This collaborative effort focuses on meeting those needs in an effective and efficient manner, and centralizing key services.
