Our office hours are 8:00a.m. to 4:30p.m. however, in order to receive service we recommend that you arrive no later than 4:00p.m.
Starting July 1, 2011, all certificates will be $20.00 and paternities, corrections, and amendments for births, which occurred from 1997 to the present, are $30.00. Paternities, corrections, and amendments are accepted Monday - Friday from 8:00a.m. to 4:00p.m. We may be reached by phone at (602) 506-6805. Se habla Espanol.
The Office of Vital Registration may issue certified copies of birth certificates for births that occurred in Arizona from 1950 to the present. Our office may issue certified copies of death, fetal death, and certificates of birth resulting in stillbirth for recent events.
Events that occurred outside of Arizona:
If you are looking for a birth or death record from another state, you must contact that state's vital records office. Birth records that occurred in other states are not available from the Maricopa County Office of Vital Registration. Death records that occurred in other counties are not available from Maricopa County Office of Vital Registration. Marriage and divorce records in Arizona are maintained by the
Clerk of the Superior Court in the county where the event occurred. Marriage and divorce records are not available from the Office of Vital Registration.
