Those with SNAP, TANF or AHCCCS automatically qualify.
If you are pregnant, the unborn baby is counted as a family member.

How Do I Apply?
Call the clinic of your choice to make an appointment (walk-ins are served after previously scheduled appointments).
When you visit, please bring the following items:
A form of identification - like a photo ID (driver’s license, state ID), birth certificate, or immunization record
Proof of address - like a driver’s license, state ID, or utility bill
Proof of income - like a pay stub or letter from AHCCCS, SNAP, TANF, Section 8 housing
Each family member applying for WIC must be present
