Amounts are based on income before taxes.
Those with SNAP, TANF or AHCCCS automatically qualify.
If you are pregnant, the unborn baby is counted as a family member.


How Do I Apply?

Visit a clinic close to you to apply (walk-ins are served after previously scheduled appointments).

When you visit, please bring the following items:

  • A form of identification - like a photo ID (driver’s license, state ID), birth certificate, or immunization record

  • Proof of address - like a driver’s license, state ID, or utility bill

  • Proof of income - like a pay stub or letter from AHCCCS, SNAP, TANF, Section 8 housing

  • Each family member applying for WIC must be present
