Maricopa County Office of Vital Registration Main Office at 3221 N 16th St. REOPENS TODAY; TUESDAY, OCTOBER 23
PHOENIX (October 23, 2012) – Today, Maricopa County’s Office of Vital Registration Office located at 3221 N. 16th St in Phoenix, resumed services after a flooding incident that forced a temporary shutdown. No personal records were compromised due to this issue.
In addition, Residents can also receive ALL vital registration services such as birth and death certificates, paternities, corrections and amendments at the satellite locations located at 3003 W. Thomas Rd in Phoenix and at 4419 E. Main St, Suite 105 in Mesa. Certificates can also be ordered online by logging in to www.WeArePublicHealth.org and clicking on Birth and Death Certificates.
“We are sorry for any inconvenience this may have caused residents,” said Dr. Bob England, director of Maricopa County Department of Public Health. “We owe a huge thank you to our employees who jumped into action at our satellite locations and all of our vendors for jumping in and helping to get our offices reopened in record time.”
