Maricopa County Office of Vital Registration Main Office at 3221 N 16th St CLOSED Until Further Notice Due to Flooding
PHOENIX (October 12, 2012) – Due to a plumbing issue that caused major flooding, Maricopa County’s Office of Vital Registration Office located at 3221 N. 16th St in Phoenix has been forced to close indefinitely. No personal records have been compromised due to this issue.
Residents can still receive ALL vital registration services such as birth and death certificates, paternities, corrections and amendments at out satellite locations located at 3003 W. Thomas Rd in Phoenix and at 4419 E. Main St, Suite 105 in Mesa. Certificates can also be ordered online by logging in to www.WeArePublicHealth.org and clicking on Birth and Death Certificates.
“We are sorry for any inconvenience this may cause residents and are working with the property owner to develop a plan for getting are main Vitals’ office up and running as quickly as possible,” said Dr. Bob England, director of Maricopa County Department of Public Health.
“We ask for people’s patience as this may mean waiting a little a longer at our satellite sites. Luckily, we have an experienced staff dedicated to customer service who will do their best to keep the lines moving,” added England.
