Permit Action Request
A Permit Action Request is a form required to begin one of the following actions:
|     Cancellation |
    Extension |
|     Renewal |
    Refund |
INSTRUCTIONS FOR SUBMITTING THE REQUEST FORM
You may enter your information onto the form from your desktop - Permit Action Request
Once the information has been entered, you may:
- Print a copy
- Fax to (602) 506-3282 - or
- Mail to the office address listed below, Attention: Tom Ewers - or
- Bring the completed form to the office
If you have Adobe Professional 7 or higher, you may enter your data and save the form onto your computer. Then
send as an attachment to pndplanreview@mail.maricopa.gov.
If you have a scanner, you may print the completed form, scan and attach to an email.
You should receive a response within 7 to 10 days.
