Food Service Worker Program

Online Testing from Approved Private Websites

Online testing is an alternative to taking the Food Service Worker test at one of the Department’s four offices; however, it does not replace the Maricopa County Environmental Health Food Service Worker Card requirements. The following is the three-step process to obtain your Food Service Worker Card:

STEP ONE - Seek an online instructional course and test from any of the following approved private companies:

  • Above Training
  • Arizona Food Handler Card
  • Arizona Restaurant
  • State Food Safety
  • Safeway Certifications
  • Arizona Food Handler

NOTE: The courses range from 75 minutes to 120 minutes and must be completed before you can take the test.

STEP TWO - Upon completion of the course and passing the online test, PRINT the bar-coded certificate in your home or office. See below for examples of approved certificates.

NOTE: We are unable to print the bar-coded certificate for you.

STEP THREE – Acquire a Food Service Worker Card by bringing your bar-coded certificate of completion and a photo ID to one of the four offices listed. The fee for the Food Service Worker Card is $5.00 and copies are $3.00. The card will be valid for three years from the certificate of completion date.

You must bring Proof of Legal Status/ID to get the card.
