Reducing your risk for heart disease is important. In fact, it's one of the best things you can do for your health. That's why Maricopa County is pleased to give you the opportunity to join this free program called Healthy at Heart Reduce Your CV Risk that is available to all county employees and their dependents.
The Healthy at Heart Program is a series of educational and support materials. Click here to view the CV Risk Mini-Brochure. The goal of the program is to teach you about and promote heart-healthy living. When you sign up, you get a full year of information, useful tools, and tips to help you manage your risk factors for heart disease.
The program also offers the Diabetes Control Network™ with tips and tools for people with diabetes. If you have diabetes, be sure to check "yes" on questions #5. And if you'd like to get tools and tips to help manage your diabetes, check the box on the back of the enrollment form.
