Diabetes Management Program
The Diabetes Management Program is administered by the Employee Benefits Division. The program provides an opportunity for employees and their dependents to be rewarded for taking control of and managing their diabetes.
The goal of the program is to assist participants with controlling blood glucose levels and modifying their lifestyle to prevent or slow the progression of cardiac complications, and diabetic-related eye, kidney, and nerve diseases.
Participants who successfully complete all 9 program requirements receive appropriate diabetic supplies and diabetic medications at no cost for one year. Participation can be continued annually by recertifying the program requirements.

Healthful Living Diabetes Care Management Program
The Healthful Living Diabetes Care Management Program, developed in collaboration with Joslin Diabetes Center and Walgreens Health Initiatives, is designed to educate members about their condition, increase adherence to their treatment plan, and thereby help reduce the potential risk of related health complications and the resulting healthcare utilization.
This program is available to employees and their dependents who are enrolled in either the Co-insurance or Consumer Choice Walgreens Health Initiatives Pharmacy benefit and who have enrolled in and met the qualifications of the Diabetes Management Program administered by the Employee Benefits Division.
Participants who successfully complete the program will be reimbursed for up to 9 diabetic-related office visit copays during the plan year.

CIGNA Diabetes & Nutrition Services
Your CIGNA medical benefit includes 3 visits per plan year with a nutritionist or dietician or to attend classes for your primary care office visit copayment. Refer to the CIGNA Diabetes and Nutrition Services link below for information on how to make an appointment and available classes.
