Blueprint for Wellness™ is a program that provides you with results from more than 30 lab tests, including the prostate antigen (PSA) test for men over 40, all through a simple fasting blood test. A few weeks after having the lab specimens processed by Quest Diagnostics labs, you’ll receive a personal lab report that shows your lab values and how they compare to normal ranges. Most doctors don’t provide you with the amount of test results you’ll receive from this worksite wellness event. Anyone who has participated in “Blueprint” before will tell you that this is one of the “best kept secrets” in Maricopa County.

You are eligible to participate (employee only) in this free screening if you are enrolled in any of the County sponsored medical plans and have not participated in "Blueprint" in the last 6 months.

During an open registration period (September 6th to September 24th),
to make your appointment, go to:

  1. https://my.blueprintforwellness.com
  2. Enter MAR2229 for the registration key as a New Participant.
  3. Enter your employee ID and date of birth and click submit.
  4. Verify your information and create username/password and click submit.
  5. To schedule your screening, click Continue.

Tests Include:

Heart Blood Tests: Triglycerides, Total cholesterol, HDL (high-density lipoprotein) cholesterol, LDL (low-density lipoprotein) cholesterol, Chol/HDLC Ratio, and Cardio CRP (C-Reactive Protein)
Thyroid Blood Test: TSH (thyroid-stimulating hormone), 3rd Generation
Kidney Blood Tests: Urea Nitrogen (BUN), Creatinine, and EGFR (epidermal growth factor receptor)
Bone Blood Test: Calcium

Pancreas Blood Test: Glucose
Liver Blood Tests: Direct and Total Bilirubin, GGT (gamma-glutamyl transpeptidase), ALT (alanine aminotransferase), Total Protein, Albumin, Globulin, Albumin/Globulin Ratio, Alkaline Phosphatase, AST (aminotransferase aka serum glutamic)
Other Blood Tests: Uric Acid, Total Iron, Iron Binding Capacity, % Saturation, Sodium, Potassium, Chloride, and Ferritin

Haven't received your Lab Results?

In order to view your Lab Results, please log into your account at https://my.blueprintforwellness.com.

If you haven't before, enter your Maricopa County Employee ID Number (9 digits starting with 81) and use the Registration Key Code MAR2229 to create your account. If you have registered before, enter your user name and password.

Once logged into your account you will be able to "View Your Results"

Your Lab Results will also be generated and mailed to you. To check the address on file, click on the link labeled "Edit My Profile" under the "My Account" section.

NOTE: April 1, 2011 will be the last day you will be able to view/print your Personal Lab Report.
