An HCH Outreach Team, which consists of a Medical Provider and a Social Service Staff worker, conducts aggressive street outreach at shelters, congregate meal sites, faith-based organizations, canals, river bottoms, and the desert. The Team provides medical services, referrals for additional medical treatment, and social services including substance abuse and mental health treatment, case management and housing assistance. Socks, underwear, hygiene kits, water and Gatorade are distributed. During the summer, when Arizona experiences extreme heat conditions, HCH takes extra precautions to keep the homeless hydrated. Staff distributes water and Gatorade to all clients in the clinic and to persons around the Human Services Campus. Likewise, HCH distributes blankets during the cold winter months. HCH has volunteer opportunities available for persons wanting to assist the HCH staff with outreach activities.
