I've lost my child's immunization record. What can I do?
- If your child received his/her previous immunizations in Arizona, our offices should be able to access his/her electronic immunization record at any of our five (5) clinics.
- You may request a copy of your child’s Arizona immunization record by calling our offices Monday-Friday 8am – 4pm.
- If your child received his/her previous immunizations out of state, please contact your previous health provider and request a copy of the immunization record.
- Immunizations from all states and countries are important, legal health records. Please bring ALL of your child’s records with you when visiting any immunization clinic.
What personal forms of ID do I need to bring for my child to receive immunizations?
- Only your child’s immunization record.
Why does my child need immunizations?
- Immunizations are the MOST EFFECTIVE means of stopping the spread of disease.
- Washing your hands after using the restroom, before eating and handling food, and after coughing or sneezing are also very effective measures to stop the spread of disease.

- Vital Records (birth certificates for children born in Arizona).
