Clinic and Immunizations
Immunizations and anti-malarial medications are available, for a fee, for people traveling to foreign countries where vaccine preventable disease and malaria are common. To learn more about which vaccines are required for your travel destination, visit the Centers for Disease Control and Prevention. You must schedule an appointment to receive foreign travel immunizations. Please call (602) 506-6909 to schedule an appointment. Calls will be returned in one to two business days. - There is a first-time foreign travel consultation fee of $40.00, or $42.00 if your visit requires a prescription. For subsequent visits, there is a $21 adult immunization fee.
We accept the following forms of payment: cash, MasterCard, or VISA. Sorry, no personal checks accepted.
Do I need a prescription to get immunized?- A prescription is needed for medications to prevent malaria. You can get it from your personal physician or the Public Health Clinic physician for a fee. The other immunizations do not require a prescription.
What about childhood immunizations?- If your child has received all of the recommended vaccines for children, they do not have to be revaccinated against childhood diseases when traveling abroad. However, it is recommended that you verify that your child has received all of the necessary vaccines needed for the area in which you are traveling. There are countries where polio, measles and other childhood diseases are prevalent.
Children under 18 years of age should visit their pediatrician to update their immunization record. Please bring all immunization records to your travel appointment.
Children under age 18 should be accompanied by a parent or legal guardian.
Where do I get immunized?- You can get all of the foreign travel immunizations at the main Maricopa County Public Health Clinic located at 1645 E. Roosevelt Street, in Phoenix.
Our clinic is open most days of the week. However, you must make an appointment by dialing 602-506-6909.
Are laboratory services available?- No. Visit your primary care physician or consult an infectious disease medical doctor for any post-travel illness or concerns.
How do I pay for the immunizations?- We accept cash, VISA, MasterCard, pre-approved business checks and debit transactions. We're sorry, but we do not accept personal checks.
LOCATION:- We are located at 1645 E. Roosevelt Street in Phoenix, just south of the I-10 and 202 freeway interchange. If you are traveling from the east, exit off the 202 westbound at 24th Street. Head south on 24th Street, then turn west on Roosevelt Street. We are located on the south side of Roosevelt Street, east of 16th Street. If you are traveling from the west, exit the I-10 eastbound at 16th Street. Head south on 16th Street, then turn east on Roosevelt Street. We are located on the south side of Roosevelt Street, east of 16th Street.
Medical Records- Copies of your medical record(s) can be requested for a fee of $6.00 per record.
Release forms are available for download or by calling (602) 506-6018. Release forms are also available at the Medical Records office, located at 1645 E. Roosevelt Street in Phoenix. Office hours are Monday through Friday, 7:30 AM to 5:00 PM.
