Adult Immunizations

Adults may receive immunizations at our clinic, located at 1645 E. Roosevelt Street in Phoenix. We accept walk-ins on Wednesdays and Thursdays from 1:00 PM to 4:00 PM, or call (602) 506-6068 to schedule an appointment.

There will be no foreign travel consultation and/or vaccination provided during the walk-in clinics.

  • There is a $21.00 adult immunization consultation fee per patient per visit.
    We accept the following forms of payment: cash, MasterCard, or VISA. Sorry, no personal checks accepted.
Rabies
  • Maricopa County Department of Public Health works closely with the Arizona Department of Health Services to evaluate exposure and risk in individuals exposed to rabies, in order to determine the client�s vaccination needs.

    Rabies vaccines for both pre-exposure and post-exposure are available at our clinic, located at 1645 E. Roosevelt Street in Phoenix. No rabies immune globulin available.


Medical Records
  • Copies of your medical record(s) can be requested for a fee of $6.00 per record.

    Release forms are available for download or by calling (602) 506-6018. Release forms are also available at the Medical Records office, located at 1645 E. Roosevelt Street in Phoenix. Office hours are Monday through Friday, 7:30 AM to 5:00 PM.

    Completed release forms can be faxed to (602) 506-5079.
