Dental Insurance

The Office of Oral Health administers a dental insurance program for residents of Maricopa and Pinal County who are living with HIV/AIDS and have an income that is 185% or less of the Federal Poverty Income Guidelines.

Program guidelines, instructions, enrollment and release of information forms are available.

Dental Sealant

The Office of Oral Health administers the Arizona Dental Sealant Program in Maricopa County. This program provides dental sealants to AHCCCS, KidsCare and uninsured second and sixth grade students from schools where 65% of the students participate in the Arizona Department of Education National School Lunch Program.

School nurses within Maricopa County who are interested in having their school participate in the Arizona Dental Sealant Program are invited to call (602) 506-6946 for additional information, or (602) 542-1866 to request an application form.
