Senior Citizen	Sterilization certificate for proof of sterilization required. Must have proof of current rabies vaccination. Must have identification for proof of age. Limit two dogs per household under reduced fee (additional dogs will be licensed under full price). Must purchase the first license for the reduced fee at one of our Centers.	$6.00
NOTE: Late fees are $2.00 per month for sterile dogs and $4.00 per month for non-sterile dogs.

PROOF OF STERILAZATION (SPAY/NEUTER)

Acceptable forms of proof of sterilization:

• Spay/neuter certificate from veterinarian that completed the surgery
• Certificate from veterinarian who has examined your dog verifying altered status
• Previous license information showing spay/neuter status

LOST LICENSE TAG

If your dog loses his license tag, you may purchase a replacement for $4.00. Send a check with a note stating that your dog lost his tag and include the tag number from your original receipt. If you can’t find the receipt, please include your name, address, phone number and the dog’s name and description. You can also come into one of our centers to get a new license tag.

TRANSFER LICENSE TAG

If you adopted a currently licensed dog from its owner, you do not have to purchase a new license. Simply apply for a transfer of ownership at any of our centers or by mail. To apply by mail, send a written and signed request, the original license receipt (or the dogs license number) and a $4.00 transfer fee to the address below:
