Testing Sites & Hours of Operation

All offices will be closed from 9:00am - 1:00pm July 9th.
The offices will re-open at 1:00pm.

Testing is conducted on a walk-in basis at our four locations. Allow enough time to complete the test. All exams are picked up at 11:30am and 4:30 pm No testing is done on holidays. For further information, please call 602-506-2960.

FEES & ADDITIONAL INFORMATION

The card fees are as follows: food service worker card $16.00, certified manager card $5.00, duplicate copies with original are $3.00 each. Volunteer cards are free. Your card must stay with your employer for the duration of your employment. The cards are valid for 3 years. Once your card expires, you must retest to obtain a new card.

NOTE: We do not keep records of your card in our computer. Therefore, if you lose your card or it expires, you will have to re-test in order to get a new one.

Please have exact change. Cash only. No bills larger than $20.00 will be accepted.

Credit cards, debit cards and checks are NOT accepted.

COSTO E INFORMACIÓN ADICIONAL

El costo de las tarjetas: Tarjeta de trabajador de servicio de alimentos es $16.00, La tarjeta de Manejador Certificado $5.00, Copias adicionales con original $3.00. Solo aceptamos efectivo. Tarjeta de voluntario es gratuita.

Su tarjeta debe mantenerse en su lugar de trabajo. La tarjeta es valida por 3 años, ya que se vence tendrá que tomar el examen de nuevo para obtener una tarjeta nueva.

NOTA: No guardamos registros de su tarjeta en nuestras computadoras. Por lo tanto, si pierde su tarjeta o expira, tendrá que volver a tomar el examen para obtener otra.
