CIGNA Medical Group Diabetes Education Program & Nutrition Services
Your CIGNA medical benefit includes 3 individual appointments per plan year with a nutritionist or dietician for your non-CCN specialist office visit copayment, or you may attend group classes for your primary care office visit copayment. Employees and their covered dependents enrolled in the Choice Fund medical plan will be subject to the annual deductible and co-insurance.
Individual Visits
If you are enrolled in the CMG High or Low plans, a referral from your Primary Care Physician is required before making an appointment.
The non-CCN specialist office visit copay applies to each individual visit for members of all CIGNA medical plans, except for Choice Fund. Choice Fund members pay either the full cost for each visit which is applied to their annual deductible, or, if the deductible has been met, a 10% co-insurance.
Group Classes
If you attend a group class, your primary care office visit copay applies except for Choice Fund. If you are enrolled in the Choice Fund medical plan, services are subject to the annual deductible and co-insurance.
