Open Enrollment Survey for all Benefits-Eligible Employees: 
Short and to the point. That’s how our Open Enrollment survey is designed. It won’t take more than two minutes to complete, and the input you provide will be timely and invaluable.  It will help us learn what worked and what we can improve on for our next Open Enrollment cycle.

To take the survey, click here.  You can only take the survey once, and the link will close on Friday, May 18. All responses will remain confidential.

Open Enrollment:
Open Enrollment ended on May 4th. On May 10th, a Confirmation Statement will be mailed to all benefits-eligible employees to their home address. Review the Confirmation Statement to confirm that your benefit elections were recorded correctly for FY13. If they were not, log on to the ADP portal at  https://portal.adp.com, access the Open Enrollment link on the Benefit Enrollment System, and make any corrections needed. All corrections must be made by no later than 5:00 pm on Friday, May 18, 2012.

Another Confirmation Statement will be mailed to you on May 24th, if you made corrections between May 5th and May 18th.

 The benefit plans become effective on July 1, 2012.

Biometric Screenings:
Biometric Screenings will be conducted on a walk-in basis at the Cigna Care Today Clinic at 102 N. Central Avenue in Phoenix. The hours are 8:00 am - 6:00 pm Monday - Friday.
